Our Parker Navigator program has been designed for seniors who are active, live at home, and value their independence. Our goal is to provide seniors with helpful information and senior-friendly resources to support them to thrive in their home. When a senior joins the Parker Navigator program, they are matched with a Care Navigator, a healthcare professional who is knowledgeable of senior services. Working closely together, a uniquely designed plan is created to address the senior’s needs and preferences.
Our Parker Navigator program is also available to support caregivers. Caring for a loved one at home can present its own unique challenges. Please know that you are not alone and that we are here to help! Our Care Navigators are specially trained and ready to provide tips, strategies, and education to support caregivers on their journey.
